(Services are subject to a fee on a sliding scale)
Health Services Include:
- Health Check Exams (ages newborn to 21 years with select Medicaid coverage)
- General Assistance Medical Program (GA-MP) (for eligible and assigned members only)
- Wisconsin Well-Woman Health Screening Program (must meet eligibility requirements)
- Limited onsite laboratory and diagnostic services (fees must be paid in advance)
- Information and Referral Services
If you have a provider, we encourage you to call them for an appointment. Nursing serves provided by the University of Wisconsin-Milwaukee, School of Nursing. Medical services provided in collaboration with Covenant Medical Group. Effective 9/01/04. Hours are subject to change.
